Do you accept insurance? 

For physical therapy services, we are out of network with all insurance companies, except for Blue Cross Blue Shield of Texas (BCBS) and Medicare. For patients who use the self-pay option, we provide superbills upon request for patients to submit to his or her insurance company. Historically, depending on the insurance benefits, many patients have received up to 100% for services rendered.

For functional medicine services, we do not accept insurance. We do offer flexible payment plans, accept HSA’s, and are partnered with CareCredit for financing options.

We are in-network with BCBS, United Health Care, Cigna, and Aetna for our professional counseling services only. 

Do you offer general physical therapy?

Yes. We provide physical therapy for generalized orthopedic conditions, such as joint pain, post-surgical conditions, fractures, muscle strains, ligament injuries, and more. Please refer to our section on orthopedic physical therapy for more information about this service.

Do you perform an internal examination for pelvic floor therapy?

A vaginal or rectal internal examination is highly recommended to accurately assess the pelvic floor. However, it is only performed with patient consent and with medical clearance.

Do you treat men for pelvic floor therapy and functional medicine?

Yes. At 360 Health and Wellness, we also offer pelvic floor therapy and functional medicine services for men.

How many visits will I need?

Every patient is different. The average number of visits for pelvic floor physical therapy or orthopedic physical therapy is 6-12 visits. However, the number of visits can vary due to the nature of the patient’s condition.

Do you offer telehealth?

Yes. We offer telehealth services for pelvic floor physical therapy, orthopedic physical therapy, functional medicine, and professional counseling services.

What is your cancellation policy?

We require a 48-hour notice for any rescheduling or cancellation, or $75.00 will be charged to your account.

In the event that there is an emergency or illness of the patient preventing a 48-hour notice for rescheduling or canceling, we require a phone call to our office to reschedule at the time of cancellation, and/or a valid doctor’s note, in order to avoid a $75.00 charge to the account. Emails and/or messages sent in the portal for rescheduling or canceling for any reason with less than 48-hour notice versus a phone call to our office will not be accepted, and, will be subject to the cancellation fee.
